Well I read a post on WA today that sheds some light on why the bindings for my third and fourth mouse buttons.
Apparently the game will only recognise one mousepress at a time so it looks like it's a bit temprmental if it will recognise the press or not, but if you're moving about and using the right-mouse button to face then any other mouse buttons will be disabled. Annoying but can be worked around if you release before you press another button.
